Community Health Network Embraces New Providers

Alvin, Texas, December 23rd, 2021 — Community Health Network (CHN) has expanded its team of providers! These new providers bring a patient-centered preventative care approach. In addition, the new providers specialize in family care. These new additions will help CHN to continue providing quality patient-centered care to more patients. Furthermore, their expertise fortifies the services and professional standards at CHN.

Tran Nguyen, MSN, RN, APRN, FNC-C

Tran Nguyen is a Board-Certified Family Nurse Practitioner by the American Academy of Nurse Practitioners. Nguyen began her career at Ben Taub General hospital in the IMU unit, then later worked at Houston Methodist Hospital on the telemetry unit for three and a half years. While working full-time at Methodist, she obtained her master’s degree in nursing from UT Health Science Center at Houston. Nguyen has been a Nurse Practitioner since 2018, working with patients of all ages. She can treat a variety of acute and chronic conditions. Her goal is to promote healthy lifestyles to prevent diseases and their complications. Tran Nguyen will be joining us at our Scarsdale and Pearland locations.

Anneliese (Onie) Hulse, MSN, APRN, FNP-C, 

Anneliese (Onie) Hulse is a Board-Certified family nurse practitioner. She was certified by the American Academy of Nurse Practitioners licensed to practice in Texas. She began her career with a bachelor’s degree in nursing from the University of Texas Health Science Center School of Nursing in Houston in 2015. Her nursing experience includes labor and delivery and antepartum nursing at The Woman’s Hospital of Texas, Houston Methodist, and Clear Lake Regional. Hulse graduated with her master’s degree in nursing from UTHealth Cizik School of Nursing in Houston in 2021. She is passionate about preventive health, evidence-based, patient-centered care, and building lasting patient relationships with those she serves. Anneliese Hulse will be joining us at our House and Northern locations.

Community Health Network is a system of federally qualified health clinics located in Brazoria, Galveston, and Harris Counties that provide pharmacy, primary care, pediatrics, women’s health, men’s health, senior health, oral health care, psychiatry, and therapy.
