Coordinating Your Health
CHN provides integrated care management services for a higher level of coordinated care. We collaborate with your primary care and dental providers, therapy and psychiatry specialists, care coordinators, case managers, as well as with your family.
Our care coordinators will maintain monthly virtual or in-office visits which caters to your schedule and needs. During these visits, your coordinator will review your care plan to evaluate your overall progress, needs, and overall health.
