How MyCHN is Addressing Non-Medical Drivers of Health?
MyCHN is committed to addressing the nonmedical drivers of health by expanding access to essential services and reducing barriers to care. We offer virtual medical and behavioral health visits to eliminate transportation challenges, along with a wide range of in-house programs such as Title V, Family Planning, and sliding scale services for medical, dental, and behavioral health needs.
Our onsite and mail-order pharmacy services further enhance access and affordability for medications. While MyCHN provides many comprehensive services, we recognize that patients may need additional support. Through our referral system, Elevations, we connect patients to community resources including utility assistance, food pantries, clothing support, and more.
