Community Health Network Increases Medical Provider Force In the Southern Region Press Release

Angleton TX, September 17th, 2021—Community Health Network is expanding once again: this time, new providers have joined the team. With them, they bring a holistic, patient-centered approach to practice at various CHN locations. The new providers are certified in a variety of fields ranging from dental, family medicine, and pediatrics. Their expertise fortifies the services and professional standards at CHN. The growth in providers, furthermore, affords CHN an opportunity to provide its services to more patients while maintaining its commitment to offering quality care.

Destanee Hamilton (MPAS, PA-C) 

Destanee Hamilton received her Bachelor of Science in Biology from Baylor University in 2018; afterwards, she continued her education at the University of Texas Southwestern, where she earned her Master’s in Physician Assistant Studies in 2021. Upon graduation, Hamilton began her career as a Physician Assistant at CHN’s Freeport Family Health Center clinic. With a strong passion for patient advocacy, preventative health, and chronic disease management, Hamilton will continue practicing in the Southern Region.  

Jessica Evans (MSN, APRN, FNP-C)  

Jessica Evans received her Associate Degree of Nursing from Brazosport college in 2012, and a Bachelor of Science in Nursing from University of Texas of Arlington in 2017. After practicing for many hours as an emergency room nurse in Houston and Brazoria County, Evans furthered her education by receiving her Master of Science in Nursing from the UTA. Upon graduating, Evans joined Dr. Brian Feaver’s team at Feaver Family Practice in Lake Jackson. Now, Evans will be practicing at the Southern Region where she is passionate about patient education and preventive health.
