CHN’s Partnership with T-Mobile Press Release

Angleton, TX, May 10, 2021 — Community Health Network (CHN) has partnered with T-Mobile to offer its virtual care and remote patient monitoring to patients that lack access to network-enabled devices or network service.

“This is one of the most exciting projects we have developed here at CHN. This is a critical step toward the future as we help people stay healthy and independent live,” said Mark Young, Community Health Network’s Chief Executive Officer.

Remote patient monitoring includes Bluetooth-enabled blood pressure cuffs, glucometers, scales, and pulse oximeters. CHN chronic care managers will monitor patient readings and ensure appropriate care is made available. This system will also support virtual visits for medical and behavioral health care. The Health Resources Services Administration and Episcopal Health Charities assisted in funding this project for patient home health devices and chronic care managers for staffing. The Brazoria County Hispanic Chamber of Commerce was a key stakeholder in facilitating the partnership with T-Mobile.
