Lisa has a patient who lives alone, and unfortunately, her living situation has deteriorated to nearly unlivable conditions. The house had only one sink with minimal running water, and the patient’s house was very cluttered and unclean. She was very hesitant even to let us come to her home and visit her due to the condition of her house. Our Home Health team went to work and arranged for the patient to have home health aide visits to assist with bathing, physical therapy for a previous shoulder injury, and skilled nursing assistance to help with medication compliance and education. The patient has been making significant strides with each visit. Just having regular interaction with people had improved her mental and physical health. She was getting stronger, walking further, and smiling more with each visit. One day after a visit, the patient’s neighbor saw Lisa outside. The neighbor inquired about the patient as she knew the patient lived alone and had minimal contact with family. Lisa mentioned that the patient struggled with social isolation and could benefit from someone checking on her. Lisa also noted that the patient was seeking help with household maintenance. The neighbor then began to check on the patient. Today, the patient called me overjoyed because her neighbor had sent over a plumber who was able to fix her kitchen sink and shower, which was no longer running, at no cost to the patient. The patient also shared that the neighbor’s sister had a cleaning service that would help with cleaning her house. She was so thankful for everyone’s assistance. She said that she had been crying and praying recently and that we had helped to answer her prayers.
