Jordana Harwell went significantly above and beyond for a patient in need of documentation in order to receive the formula needed for an NG tube. She went to extreme measures and kept in contact with my Care Coordination, the patient’s Mother, and the provider to assist in getting the job done. She understood that the task needed to be completed no matter what and did not stop until it was taken care of. She stayed in contact with my supervisor and me through the whole process and provided detailed updates every step of the way. In the end, the patient was taken care of, and his Mother was very appreciative. Without the documentation and visit with the provider Jordana set up, the patient was at severe risk of ending up in the hospital again. Jordana also made sure it was possible to be seen very fast, and she followed up with the patient’s Mother several times. She also suggested that we offer the Mother more support in the form of Care Coordination because she is going through so much.
