Due to her high-risk social situation and imminent delivery, immediate assistance was required. Ana assisted the patient by identifying and connecting her with local women’s shelters to secure temporary housing. Additionally, transportation resources were arranged to ensure the patient could attend her prenatal appointments. The patient was provided with community resource information and support to promote stability and continuity of care.
