Virtual Visits Are Required For Anyone Seeking COVID-19 Testing

Community Health Network (CHN) is offering COVID 19 antigen testing for $15, PCR testing for $85, and virtual provider visit with antigen testing for $55 to uninsured/self-pay patients effective immediately.
CHN will provide a Virtual Visit prior to receiving COVID-19 services. The medical provider will first provide a virtual visit through Doxy.Me and Zoom and order a Flu test, Strep test, and or COVID test, if needed for the patient.
Pre-Visits: Medical Assistant calls the patient and conducts the health/COVID screen and completes intake.

Screening and Testing

As a leader in providing Primary Care Services, Community Health Network remains committed to our community’s health and well-being. Community Health Network has taken all necessary steps to ensure that we are responding to the needs of our community by expanding our testing capabilities. While we are not conducting routine COVID-19 testing, we are providing COVID-19 screenings, according to the Centers for Disease Control guidelines.

Individuals wishing to be screened may call our health center. We have outlined a detailed process for our community to help them stay informed on how they can be tested if they meet the screening requirements.

Virtual Visits

To ensure and promote social distancing, we offer virtual appointments for individuals who meet Covid-19 screening requirements. To schedule a virtual screening, please call our center. In addition to our screening services, you can also set up virtual medical, pediatric, and women’s health visits, psychiatric sessions, and counseling services. If you use CHN as your pharmacy, your prescriptions will be mailed at no extra cost.

Testing Centers

To protect our patients and staff, we’ve implemented designated screening and testing centers such as Freeport Community Health Center in Freeport, Adoue Family Health Center in Alvin, Pearland Family Health Center in Pearland, Scarsdale Family Health Center in Southeast Harris County (South Belt area), and Bay Area Family Care in Bacliff.

Please note – COVID-19 testing is only available for prescreened and scheduled individuals only.
