CHN Launches Virtual Remote Preventative Care Services Press Release

Alvin, Texas, February 22nd, 2022 —Community Health Network is thrilled to announce the launch of CHN@Home in March 2022. CHN@Home puts patient-centered care in the hands of the patient. MYCHN will offer whole-person preventative care at home. Whole-person preventive care at home will allow patients to receive virtual care in their homes. Patients will also utilize telemedicine appointments to have their care needs met.

Patients will access remote patient monitoring services (RPM). RPM services allow patients to send the CHN@Home care team readings of blood pressure, glucose monitoring, pulse oximetry, and daily weights. Remote patient monitoring is performed via tablets, Bluetooth-enabled blood pressure cuffs, glucometers, scales, and pulse oximeters into RPM services. Along with RPM devices, patients who do not have internet access will also be provided with network services. This and RPM services are a result of MYCHN’s partnership with T-Mobile. In addition, patients will have their electronic medical records updated upon using RPM devices.

CHN@Home allows patients to take more control of their preventative healthcare experience. Patients can receive preventative care in the comfort of their own homes. As a result, patients can save time and focus on what matters to them. MYCHN is delighted to be at the forefront of making healthcare services even more accessible for the communities we serve.
